Old Cairo is a part of Cairo so don't worry. Cairo usually refers to the Greater Cairo area which constitutes the entire Cairo city-province, besides parts of two other provinces (Giza, Qalyoubiya) and two whole provinces that recently spun off Cairo (Helwan, Sixth of October).
So yes, Old Cairo is more like a district of Cairo. It's right in the middle of downtown Cairo
