I start off with one and a half coal peices (golden QL for now) at the very edge and it starts off just fine. As it goes along, I take one coal peice off and start moving the rest little by little to the center. I make sure it's not pushing the foil down. I don't know, it's not uncommon for me to find a little bit of shisha seared to the foil (even when smoking SB), but I don't have more than a minute or two of harsh smoke before it resolves itself. In my experience harsh smoke is caused by too much heat. If you feel like you're not using too much heat, try less shisha than you're better judgement would want.
