You will be better off just replacing the stem, or hookah for that matter. If your looking for a small one like that I would suggest getting a MYA for your next one.

What i would do is just sand that off. Its just surface rust, nothing big. But i would get a new hookah too. Just make sure to clean all of the rust off of it and make sure that it is dry before and after cleaning/smoking

Yeah its just exterior rust.. use something with acidic acid such as coke.. lemon juice.. vinegar.. etc and put some on there .. let it sit for about 5 min then scrub it off and rinse it off with water

I'd try and sand it off but then eventually I'd also say the next step for that pipe is the trash heap and pick yourself up something better, preferably a small MYA.
Something like this would be a good place to start, it's built to last:

Yes. But keep in mind even the best quality metals will crap out if you dont take care of them. Keep them clean and dry between uses and you will get good life out of them. Bambino, QT either would be great.

you could easily sand it off but you'll still taste it just the same, if it's an issue, well for me it's a huge problem, you unfortunatly have to buy a new hookah ( only trust well known brands that have been tested by many like mya and km (even expensive hookahs can be crap, i spent 140 on a ice torrent hookah, and had the same problem) my recomendation, read the hookah lamp forum under the homemade section, at least that way its not a complete waste, thats what i did.

i just modded my hookah and eliminated the rusting core. the base is an old cd stack case thing.the down stem is the thing cds slide on to and i used the aluminium up stem tray and bowl, i also used a funnel bowl mod made from rockstar cap. smokes 10X better!!

thanks for all the advice!!
