The only thing a diffuser really did for me that I noticed is make the pipe significantly quieter.

It does make for a slightly smoother smoke, as well as makes the pipe produce less noise. It also helps keep the floating ash tray from rumbling.

Some people claim it also cuts down on nicotine. I've only ever used a diffuser back when I smoked washed tobaccos mainly so I can't attest to that. But as mentioned it will make everything significantly quieter which is nice if you watch movies or listen to music. I prefer not to use them because I like the sound of the pipe and also am not bothered by nicotine in the least.

my observations on the Heba used with small, medium, and large hookahs...

slightly smoother smoke -- more apparent when it gets harsh. there's a window when it's bearable with the diffuser, unbearable without it. it makes a slight difference.

slightly thicker smoke -- don't know the details of how or why. even smooth smoke, when very dense, can make me cough. the smoke is slightly thicker with one.. and it helps me tolerate it more.

significantly quieter -- big difference in gurgle volume with and without the diffuser.. using the same water level. if your water is only 1"-2" above the down-stem, it nearly silences it, i.e. no more hubbly bubbly. the gurgle gets louder if you increase the water level.. if you want to keep it : )

more restricted draw --
i opened up the Heba holes after first use. draw is still perceptibly tighter but nowhere near when it was.. a virgin. this may not be an issue with some bigger hookahs, but i expect it is with most.. depends on what you have.

This helps TREMENDOUSLY with the coals moving around from the rattling vibration. Increased my KM joy by at least 68%

IMO its worth the buy, i think it made the smoke a lil thicker and smoother, and increase in taste too. and yes it cuts down the noise. i recommend it, and the draw is exactly the same.

i revisited my diffuser on my mya diva about a week ago.

i hated how it restricted the pull so i made some adjustments. i slipped it on the stem, enough to where it'll stay on, then i screwed the stem down to my vase so that the diffuser gets pushed up according to how much i screw in the stem and left it at that (the instructions say differently).

it's like i'm not using any water, no sound, no rattling from bubbles and the pull is still the same. i kinda like the rumbling sound though.

Like others have said it cuts back on the noise and makes the smoke a little smoother. Thats about it. Personally on my KM I love it because it stopped the tray rattle
